Welcome to the Team Performance Assessment.

The purpose of this assessment is to determine the overall performance of  your team.

As we collect responses from other teams, we will share with you the average results of all teams, so that you can compare how your team is doing relative to all teams.

If the assessment is coordinated by your team leader or administrator she/he will provide you a Team Name. Please enter it in the respective field, so that we can consolidate all responses for your team.

Your responses will be kept completely confidential and will not be shared with anyone.

For each of the following questions rate your team on a scale of 1 to 5, where Strongly Agree means the best performance, and Strongly Disagree means the worst performance. 

Note that when we refer to PROJECT we mean all types of teams including operational teams such as Sales Team, Supply Chain Team, R&D Team, Customer Support Team, etc.

Also note that the order of the questions does not follow a "logical sequence" because we wanted you to consider each question independently.

Our Team Lead actively invites ALL members of the team to share their ideas and opinions at group meetings and one-on-one.

Our team members respect each-other's ideas and opinions irrespective of seniority, gender, ethnicity, experience, etc.

My Team Lead encourages our team members to take prudent risks, and tolerates failures.

Our Team Lead openly admits when she / he does not know the answer, or when she / he makes a mistake.

I feel safe and empowered to express my ideas and views even if they are contrary to those of our Team Lead's and other team members.

Our Team Lead gives us constructive feedback on our performance, and is not afraid of difficult conversations.

Our team members trust each other to deliver on our commitments and to uphold the values and behavioral norms of the team.

As much as she/he can, my Team Lead assigns me meaningful work that I like doing, that is aligned with my capabilities, and that contributes to my professional growth.

Our Team Lead continuously communicates the importance and impact of our project. Our team members are motivated by the knowledge that our project has an important impact on our company and its stakeholders.

The Objectives, Scope, and Approach of our project or operation are clear to all our team members, and have our complete buy-in.

Our team members are always results-oriented and focused on producing the outcomes and deliverable assigned to them on time, within budget, and with the quality standards agreed upon.

Our team members do not shy away from constructively discussing conflicting opinions, approaches, and solutions to the work in front of us.

Our team members always take full accountability of all aspects of their work, including successes, challenges, errors, and failures (if any).

Our team members understand and always follow our project's Methodology, Processes, Policies, and Procedures.

Our team members always strive to produce creative and innovative solutions to all aspects of our work.
