The purpose of this assessment is to determine the overall performance of your team. As we collect responses from other teams, we will share with you the average results of all teams, so that you can compare how your team is doing relative to all teams. If the assessment is coordinated by your team leader or administrator she/he will provide you a Team Name. Please enter it in the respective field, so that we can consolidate all responses for your team.
